Mayfield Preschool/ Head Start/ Early Head Start  Application Form Links: Please complete all six forms below.  Remember, once you start a form you must complete it. It will not save your information for you to return to. You will have to start the form over if you can not complete it. You do not have to complete all the forms at once but please have them completed by your registration date. Please call 270-247-8841 if you have any questions. 

Please bring the following when you come: Child’s Birth Certificate, Proof of income, Child’s Medical card and Social Security card
